Holistic Management
of Type 2 Diabetes

A 48-year-old male with a history of type 2 diabetes over the past five years faced challenges in managing his condition effectively. Despite adherence to medication prescribed by his primary care physician (PCP), his HbA1c levels remained alarmingly high, fluctuating between 12 to 11.2%, far above the target range of less than 5.6%.

This indicated a significant issue with blood sugar control. The patient, determined to improve his health, expressed a keen interest in exploring holistic approaches for diabetes management, weight loss, and overall health improvement.

What do you need to get going?

Upon initial evaluation, the patient presented with a Body Mass Index (BMI) of 30, signaling obesity. Laboratory tests revealed an HbA1c level of 11.2%, fasting glucose levels at 226 mg/dl, triglycerides at 279 mg/dl, low Vitamin B12 at 186 pg/ml, and a Vitamin D deficiency at 17.6 ng/ml. The patient reported struggles with diabetic management, weight gain, and neuropathy, contributing to a sedentary lifestyle.

Initial Assessment

Despite their good overall health, Patient S and J’s busy lifestyles and poor nutritional habits were detrimental to their reproductive health, with Patient S experiencing significant hormone disruptions. Comprehensive fertility evaluations which reveal no underlying anatomical medical conditions impacting their ability to conceive.

Medical Nutrition Therapy

Under Dr. Sanghavi’s guidance, a personalized nutrition plan, was developed, focusing on whole, nutrient-dense foods known to support reproductive health. This plan emphasized the importance of a balanced diet enriched with folate, iron, omega-3 fatty acids, antioxidants (such as selenium, vitamins D, C, and E), and zinc. Additionally, given Patient S’s irregular menstrual cycles and potential insulin resistance (HBA1C at 8%), stabilizing blood sugar levels through nutrition became a key focus. The couple was also encouraged to maintain adequate hydration.

Progress And Follow-Up

After eight months of dedicated adherence to their nutrition plan, both patients reported significant improvements in energy levels, stress reduction, and overall happiness. Notably, Patient S achieved a more regular menstrual cycle, an indication of improved hormonal balance, with her HBA1C levels normalizing to 4.8% and her BMI reducing to 21. Similarly, Patient J experienced enhanced well-being and vitality. Remarkably, the couple conceived naturally following this period of medical nutrition therapy
