Despite adherence to medication prescribed by his primary care physician (PCP), his HbA1c levels remained alarmingly high, fluctuating between 12 to 11.2%, far above the target range of less than 5.6%.

Upon initial evaluation, the patient presented with a Body Mass Index (BMI) of 30, signaling obesity. Laboratory tests revealed an HbA1c level of 11.2%, fasting glucose levels at 226 mg/dl, triglycerides at 279 mg/dl, low Vitamin B12 at 186 pg/ml, and a Vitamin D deficiency at 17.6 ng/ml. The patient reported struggles with diabetic management, weight gain, and neuropathy, contributing to a sedentary lifestyle.
